Moray’s Home Assessment Pathway (HAP)
The Home Assessment Pathway (HAP) is transforming how Health & Social Care Moray supports people to leave hospital and continue their recovery safely at home, with the right support in place.
Part of our coordinated Home First approach, and backed by Scottish Government funding, HAP brings key services together to assess, re-able and support people in their own homes – rather than making long-term decisions in hospital or during times of crisis.
Assessing people where they live gives a far more accurate and meaningful picture of their day-to-day lives – their strengths, what matters to them, and the support already around them. This leads to better, more personalised decisions, helping people regain independence and avoid unnecessary or overly intensive support.
HAP is designed to:
- support earlier, safer discharge from hospital
- prevent unnecessary hospital admissions
- reduce assumptions about long-term care needs and avoid over-prescribing care
- focus on recovery, reablement and independence – not dependency
It brings together a range of services – including Discharge to Assess, START, Community Response Team, Pharmacy, Community Nursing, Technology and Volunteers – into one flexible, coordinated, multidisciplinary approach.
At its heart, HAP reflects a simple but powerful belief: home is often the best and safest place to be, and the best place to plan for recovery, independence and the future.
